The information requested in this form will be used soley for the purpose of determining your elgibility for the TRIO Upward Bound program. Please keep in mind that this form is NOT the Upward Bound Admission Application. All students who complete this form will be notified as to whether they are eligible to apply for the program. Eligible students will have a meeting with an Upward Bound staff member and will be given an Upward Bound application at that time.
