OSU Affiliated Housing Incident Report
Oregon State University Affiliated Housing Program (AHP) approved facilities, also known as Participants, must meet three sets of requirements associated with Physical Environment, Behavioral Standards, and Educational Expectations. This form addresses the Behavioral Standards Guidelines described in the OSU Affiliated First Year Housing Program:
Every Participant will report to the Vice Provost for Student Affairs (VPSA), or their designee(s), via email, any situation involving medical emergencies, medical transport, and law enforcement interaction within 8 hours of the incident.
The VPSA’s designee for this purpose is the Dean of Student Life. This form is to be completed according to the Guidelines and will be reviewed by the Dean of Student Life and members of the AHP Behavioral Review Team.
